Home Telehealth and New Practice Framework Makes for a Great Collaboration

Care Coordination and Integrated Case Management (CC&ICM) is a new health care practice framework that is progressively rolling out across all VA facilities. CC&ICM assigns lead coordinators to Veterans who frequently use VA care and have a history of complex health care needs. With a lead coordinator, Veterans don’t have to reach out to multiple providers. Instead, they can contact their assigned lead coordinator who will help them manage the many parts of their VA care.

At the Martinsburg VA in West Virginia, this new practice has been very successful. They often combine CC&ICM with their Remote Patient Monitoring – Home Telehealth (RPM–HT) program to keep a close eye on Veterans’ health vitals, like blood pressure and oxygen levels, remotely.

“Home telehealth along with CC&ICM is a great collaboration,” said Felicia Jenkins, a registered nurse and CC&ICM lead coordinator “With high-risk patients, they could end up back at the emergency clinic at the drop of a hat. These programs help us to stay on top of everything and avoid emergency room visits.”
