Minneapolis VA Medical Center has opened its first primary care "Super Clinic." By incorporating the Patient Aligned Care Team (PACT) model, the clinic is helping Veterans access streamlined, timely care in one physical location.

PACT is where a medical team of primary care providers, registered nurses, licensed practical nurses and anesthesiologists work alongside mental health professionals, pharmacists, dieticians, therapists and other team members within the same area. The primary care "Super Clinic" helps deliver care to Veterans through various modalities, such as group visits, same day visits, telephone care and Secure Messaging through My HealtheVet.
