Among patients enrolled in Remote Patient Monitoring-Home Telehealth (RPM-HT) at the VA North Texas Health Care System in Dallas, four in five Veterans have high blood pressure. The staff noticed that these Veterans and their caregivers didn't know how to manage the condition at home.
A team of Dallas RPM-HT Care Coordinators – Annie Joseph, Anney Mathukutty, Shahin Monsef and Daisy Thomas – came up with an innovative way to tackle the problem. They partnered with the American Heart Association (AHA) to organize a three-part virtual educational symposium for Veterans and their caregivers. Now, AHA is recognizing their achievements.
