Relocating across the country can be challenging. Not only do Veterans have to adapt to a new home and community, but they also have to navigate the transition to a new health care team.
This was particularly true for a 65-year-old Veteran residing in the Reno VA Community Living Center in Nevada. He had recently had an above-the-knee amputation and was working with his VA care team to receive a prosthetic leg.
Using telehealth consultation, the Veteran’s care team from Nevada connected with his new team in Virginia to ensure a smooth transition of care.
