Sharing medical history with a Veteran's entire care team contributes to more comprehensive care plans and better overall patient care. A new study helped Veterans learn how to use My HealtheVet to share their VA Continuity of Care Document (CCD) to keep their community care providers informed. Allergies, medications, diagnoses and recent lab results from VA’s electronic health record are examples of medical information contained in the CCD.

Seventy-eight percent of patients in the study reported that the CCD would help them be more involved in their own care, and ninety-six percent of providers said they would want to continue to receive CCDs for their patients.
